For skilled physical therapy services, a referral from your physician is required.
Please request a prescription from your medical provider’s office and send a copy to our e-mail at: info@ksphysicaltherapy.com prior your appointment and/or provide us the prescription at the time of your evaluation.
If you’re unsure whether you need a referral, feel free to contact us — we’ll be happy to guide you based on your specific needs.
For Wellness Clients — wellness sessions are provided without a physician referral, on a self-pay basis, and are not eligible for insurance reimbursement.
